Youth Wraparound

We offer access, resources, and support for all our Grand and Jackson County neighbors to lead their healthiest lives.

FREE care coordination services for teens and families facing complex behavioral health struggles, focused on strengths and restoring hope.

Youth Wraparound Care Coordination supports youth ages 0–21 who are facing behavioral health challenges and their families. Through a team-based approach, youth and families work with care coordinators to set goals, plan for the future, and create support plans that reflect their needs, strengths, and priorities.

As part of the Community Health Programs team, Wraparound Care Coordination helps ensure that youth and families receive coordinated support. The team connects families to services, resources, natural supports, and community partners while helping everyone involved in a youth’s care work together.

By bringing together the people and supports that matter most, Wraparound Care Coordination helps youth stay safely at home and in their communities, improve their well-being, and achieve greater success at home, in school, and in everyday life.
