Our FREE services can help with anything that limits you from getting the care you need when you need it.
How do Navigators help?
Health Navigators and Care Coordinators guide clients through the healthcare system and help them find community resources. We can help with everything from finding healthcare providers, assistance filling out applications and understanding benefits, to help coordinating transportation to appointments.
We help clients build confidence and knowledge to help individuals and caregivers become more comfortable accessing resources and managing health independently. We work one-on-one with clients to define their health-related needs and goals. We help identify things that make it more difficult to have individual and family needs met.
