Care Coordinator

Do you enjoy helping people? Do you want to make a difference in the lives of your neighbors? Join our growing team! Coordinate care for Grand and Jackson County residents to increase access to care and reduce barriers to health. Outreach to Medicaid members identified as high need or high risk. Complete reporting for program activities. Hybrid in-office and 30% remote work. Requires travel throughout Grand and Jackson Counties, with mileage reimbursement.

Peer Recovery Coordinator

Are you in recovery? Do you want to use your life experience and skills in service of others on a similar journey? Join our growing team and make a difference in your community! Coordinate care for Grand and Jackson County residents to support access to recovery resources and treatment.
