Who We Serve & Where We Serve
Every day, we help individuals and families throughout Grand and Jackson counties overcome barriers to health and well-being. From young children to older adults, our clients represent every stage of life and primarily work in the service, tourism, and construction industries. They live across our rural communities and often face the unique challenges of high living costs, limited healthcare access, and financial hardship. These snapshots illustrate the people and communities we proudly serve.


Social Determinants of Health
These factors, called social determinants of health (SDOH), “affect a wide range of health, functioning, and quality-of-life outcomes and risks”:
- Health care access and quality: Providing access
to quality medical, mental health, dental, and vision care. - Economic stability: Helping people earn steady incomes that allow them to meet their health needs.
- Financial Self Sufficiency: Having enough money to cover basic living costs (housing, food, transportation, healthcare, childcare) without government aid or family support.
- Education access and quality: Increasing educational opportunities and help children and adolescents do well in school.
- Neighborhood and built environment: Creating neighborhoods and environments that promote health and safety.
- Social and community context: Increasing social and community support.
What is Self-Sufficiency?
‘Self-sufficiency’ is how much a family must earn to meet basic needs without assistance. (Colorado Center for Law and Policy 2018)
Local industry wages are generally less than the self-sufficiency level. Self-sufficiency in Grand County is 3 times more income than income-eligibility levels required to qualify for financial assistance programs, leaving many residents without access to safety-net programs.
50% of Grand County’s population live at or below self-sufficiency level. Nearly 70% of Granby and Kremmling residents live under the self-sufficiency level, explaining why we serve more residents from Granby and Kremmling than the population. (US Census 2016)
100% of our clients were under the self-sufficiency level in 2019!
Health is influenced by every aspect of how and where we live. Access to secure and affordable housing, good paying jobs and affordable healthcare are examples of important factors that can put people on a path to a healthier life. (County Health Rankings 2020)
