GCRHN In-Person Outreach at Middle Park Health – Granby

Grand County Rural Health Network will have in-person outreach at the three Middle Park Health locations throughout the month. On the first Fridday of each month, Jeanette Causey, our Care Coordination Manager, will be at the MPH – Granby  Clinic.
