Youth Wraparound

We offer access, resources, and support for all our Grand and Jackson County neighbors to lead their healthiest lives.

FREE services for teens facing complex behavioral health struggles, using an in-depth care coordination process focused on strengths and restoring hope.

Our Youth Wraparound program supports youth ages 0-21 who experience complex behavioral health challenges. The Youth Wraparound Coordinator follows a structured, in-depth, evidence-based process called High Fidelity Wraparound, which helps the youth and their families create a vision for their future. They follow a plan created by the youth and family, and build on their natural support systems.

Youth with complex needs are more likely to stay in their homes and communities as a result of the Wraparound Care Coordination process. They tend to hvae better mental health outcomes. If a crisis happens, out-of-home placements are typically short-term. They function better at home, school, and in their communities compared to those not in Wraparound.
