Community Health Programs

Direct Services

We get it. Understanding healthcare is challenging. Affording care is difficult. Our Community Health Programs make it easier to get and stay healthy.

We help with access to care, coverage, resources, and support for all our Grand and Jackson County neighbors to lead their healthiest lives. Services are FREE and offered in English or Spanish.

We have staff with specialized training and/or lived experience who can help with specific needs related to:

  • older adults and aging
  • complex health issues
  • social determinants of health
  • mental health & substance use recovery
  • youth and families with complex behavioral health needs
  • Spanish-speaking community members
Patient Navigation

Our team of health navigators and care coordinators help reduce barriers to health and human services, increase access to care, and coach clients to how to advocate for their health.

Certified Peer & Family Specialists help participants connect to care, community, and resources. They provide one-on-one coaching, facilitate recovery support meetings, and organize and attend sober-friendly events throughout the community.

Youth/families experiencing significant behavioral health concerns receive support through an evidence-based care coordination process. Wraparound Care Coordinator collaborates with team and partners.

Hispanic, Latinx, Spanish-speaking and immigrant community members get peer support to help with access to health coverage, care, education, and other community resources and support.

As part of Connect for Health Colorado’s statewide network, our certified Health Coverage Guides are here to help community members explore options for health insurance and financial assistance.

Need to see a doctor, therapist, or dentist? The ACHES and PAINS healthcare voucher program provides those who qualify with access to medical, dental, mental health, and supportive services.
