Jen Fanning, MS

Jen Fanning, hired by the Network in July 2007 as the Community Health Program Coordinator, assumed the Executive Director’s role in 2008. Her training and experience as a Community Health and AIDS Prevention agent in West Africa during her Peace Corps service gives Jen a worldview that is important each day in her work. Additional experience and training include health and racial equity, social determinants of health, inclusion, case management, marketing, environmental health, and teaching developmentally delayed children.  

Jen earned a bachelor’s in arts from Hamilton College, with a concentration in Psychology, and a Master of Science from the University of Denver in Healthcare Management, with a concentration in Strategic Leadership in Healthcare Organizations. Jen volunteered for the National Sports Center for the Disabled for seven years and the American Cancer Society for three years. She was a 2009 fellow of the Regional Institute for Health and Environmental Leadership Advanced Leadership Training Program, a 2011 fellow in the Community Resource Center’s Nonprofit Leadership and Management Training Program, and a 2013 fellow of El Pomar’s Emerging Leaders Development Program.  

Jen has served as co-chair of the Northwest Colorado Rural Philanthropy Days (NWCO RPD) planning committee in 2012-2013 and co-chair of the marketing committee in 2016-2017.  In 2019, she was appointed to Governor Polis’s Behavioral Health Task Force to evaluate and set the roadmap to improve the current behavioral health system in the state. She is also a trained Second Circle facilitator to build trusting relations and work toward real meaningful change. She is very passionate about standing for others and empowering others to stand for themselves.

In her free time, Jen loves spending time with her family and dogs enjoying skiing, hiking, stand-up paddle boarding, camping and the hot springs. She recently decided that she will greet her family like she greets her dogs, showing joy and love at every opportunity. Jen’s teammates appreciate her leadership, authenticity, transparency, and endless supply of drive for the community.

Pamela Flor-Stout

Pam is tri-lingual with fluency in Spanish, English, and Portuguese. This, along with her deep care for our community, has made her an important asset to the Network since she joined in September 2021. An immigrant from Paraguay, she has been in the county since 2007, working in the lodging and tourism industry until 2021. She graduated from Colorado Mountain College with an Associate’s Degree in Resort Management in 2012.

Pam has lived through the joys and obstacles that being an immigrant might carry. She began traveling at the age of 17, living in New Zealand as an exchange student for one year. Half of her life has been spent in the southern hemisphere with the other half in the northern hemisphere.

She helps our organization to reduce stigma, increase health equity, provide care coordination, address the needs and issues related to the social determinants of health that surrounds Grand and Jackson County Hispanic residents through the Promotora program, which she held since February of 2022. She also serves as a Certified Health Coverage Guide to provide outreach, education, and assistance to individuals and families in the community who are interested in learning more about their health coverage options. In May of 2024, Pamela became the first Health Coverage Program Manager. In this role, she will continue to serve as a Certified Health Coverage Guide, while engaging in program management responsibilities such as grant writing, reporting, program marketing and evaluation, as well as providing direct and indirect supervision and leadership to the Health Coverage Guides and Promotoras.

Pam’s coworkers recognize her eagerness to participate in discussion, offer unique insights, take initiative in projects, and, since Spanish is her first language, find her invaluable as an interpreter and health liaison. She loves being outdoors, especially snowboarding or skiing with her family, cooking dinners with different flavors, and challenging her daughter to enjoy the array of outdoor activities we are so blessed to have in the Rocky Mountains.

Christine Smith

Christine joined the Network in February 2015 as a Program Coordinator, being drawn to healthcare and client advocacy after experiencing support from the Network through her own health crisis 2 years prior. She became Operations Manager in June 2017 and was promoted to her current position in February 2021. Christine received her Bachelor of Business Administration from The University of Michigan with an emphasis in marketing. She brings a variety of both volunteer and professional experience in client services, promotions, event and project management, most recently as Group Services Manager at Winter Park Resort.  

In her current role, Christine works on special projects that align with our strategic plan to help grow and strengthen the health and wellbeing of our local community. She also oversees donor relations, marketing and fundraising, as well as the Network’s ACHES & PAINS healthcare voucher program. Her coworkers say they value her ability to express herself eloquently on behalf of the Network and its values. 

In her free time, Christine enjoys skiing fresh powder in the winter and trying to master stand-up paddle boarding on rivers in the summer. She and her husband are enjoying many new travel adventures together as recent empty nesters.

Lauren joined the Rural Health Network team in February 2023 as the Direct Services Program Director, seeing it as a meaningful opportunity to re-align her career with her passion for leadership, client and community services, healthcare, mental health, equity, and systemic change at the local and regional level.

Lauren grew up in the Denver area and learned to ski at Silver Creek and Winter Park Resort, starting her journey toward calling Grand County home. Frequently serving as a grounded and compassionate listener with friends and family, Lauren obtained a Bachelor of Science in psychology, sociology, and cultural geography at the University of Denver while living part-time in Grand County to continue her competitive snowboard career.

Lauren went back to school in 2010 to complete a master’s degree in counseling psychology through the University of Colorado Denver, a program emphasizing equity and multicultural competency across the curricula. During graduate school, Lauren lived in the county full-time, working at Winter Park Resort as the Snowboard Program Manager. Lauren went on to work as a Licensed Professional providing individual, couple, and play therapy as well as group facilitation.

In 2017 she scaled back her practice and re-joined the team at the Winter Park Resort’s Competition Center as the Business Manager, leading significant transformations in guest services, program administration, finance, sponsorship, fundraising, merchandising, branding, digital marketing, and communications.

Over the years she participated in a variety of local committees and work groups, such as the Grand Beginnings Early Childhood Council, Communities that Care, and has ongoing involvement in the Behavioral Health Strategic Planning Committee and the Grand Foundation’s H.O.P.E. (Healing Opportunities through Prevention Efforts) Fund Advisory Committee.

Outside of work, Lauren enjoys spending her time with her husband and her stepson during his visits home from college. Much of her free time is spent tending their four-legged family members: three horses and a dog.

Jennifer Tibbetts

Jennifer grew up in Arvada, CO and attended college in Lancaster, England and at the University of Colorado-Boulder where she obtained her BA in 1991. Prior to moving to Grand County, Jennifer owned a manufacturers’ representative sales agency in the power utility industry for 17 years in Denver, incorporating her many years of sales and marketing experience in various tech industries. She and her husband and two sons moved full-time to Grand County in 2018, and in 2019 she started coordinating client services at the Mountain Family Center, a non-profit resource organization based in Grand County.

She joined the Network in 2022 as Community Engagement Specialist and is excited to engage with even more of her neighbors in Grand County. In this role Jennifer works to connect, educate, motivate and mobilize the local community to address specific, community-identified issues. She calls upon her combined communications experience in both the corporate world and the non-profit sector in her work to foster and build consensus amongst diverse groups.

Outside of work, Jennifer is busy with two teenagers and loves spending time with her family, attempting yoga, and hiking or cross-country skiing depending on the season.

Amanda Uehlein, MA

With a focus on equity and social justice, Amanda has enjoyed learning from a breadth of work experience as a direct care provider for adults with developmental disabilities, a teacher’s aide in a school for children on the Autism spectrum, a meditation instructor, an emergency/crisis clinician, and an outpatient therapist for adults in community mental health. Originally from upstate New York, Amanda received her Bachelor of Arts degree from Manhattanville College. She moved to Colorado to pursue graduate school and completed a Master of Arts in Somatic Counseling Psychology at Naropa University. Amanda has many years’ experience in group facilitation and is a trained Second Circle facilitator to build trusting relationships across racial and cultural differences and work toward real, meaningful change. 

After relocating to Grand County, Amanda joined the Rural Health Network as a Care Coordinator in July 2018, became Program Manager in February 2019, and was promoted to Associate Director in January 2021. Amanda manages and oversees several programs for the Network. She deeply enjoys providing reflective supervision to staff in their work with clients and community members and is a passionate advocate for the health and wellbeing of her community. 

Amanda loves to go exploring with her husband and two cattle dogs. She believes in the power of art to drive change and support healing. Amanda’s teammates appreciate her support for others, her intentional use of language, and her innovative thinking.

Jack Dugwyler
Recovery Coordinator

Jack (aka “Mr. D) joined the Grand County Rural Health Network in April 2024, working as a Recovery Coordinator. Jack is committed to helping people in recovery find hope and healing. He is excited to be a part of an organization that provides opportunities for people to learn, grow, and live healthy, productive lives. He has a passion for community service and helping folks along their journey.

Jack grew up in Tacoma, WA and moved to Colorado in the 1980’s. He finished his Bachelor of Arts in History at the University of Colorado and has a Master of Arts for Curriculum and Instruction from Adams State College. Since 1995 he has been a teacher, coach, and dean with
East Grand School District. Recently, he has also begun to mow the grass for EGSD and enjoys spending time outdoors learning to relax and enjoy the moment.

Jack has been volunteering for the past 10 years with Celebrate Recovery in Granby. “Celebrate Recovery is a safe place to find community and freedom from the issues that are controlling our life”. This experience taught Mr. D how to take better care of himself and looks forward to helping others find and develop similar skills.

Stevie Kitterman, CPFS
Lead Recovery Coordinator

Stevie joined the Grand County Rural Health Network team in May 2023 as the organization’s first Recovery Coordinator. In April 2024, she became the Lead Recovery Coordinator, helping guide and mentor the recovery coordination team and further support program development. Her role as a Certified Peer & Family Specialist (CPFS) is to help serve the recovery community in various capacities including certified peer coaching, advocacy, and helping to identify and reduce barriers to both acute and long-term recovery.

To become a CPFS, Stevie completed over 100 hours of certified Peer approved classroom training as well as over 500 supervised hours working as a Peer & Family Specialist and Recovery Coach. She passed the exam and received an official COPA recognized certification as of April 13 th 2024. Community members have been able to successfully develop and maintain a long-term recovery journey working with Stevie as a support and resource broker. She is so passionate about people and seeing the light of hope return to someone’s eyes.

Stevie has been devoted to and volunteering within the recovery community for over 15 years. During her time as a Salvation Army volunteer, she facilitated workshops, supported educational and entertainment outings for women in the facility, and helped support various spiritual activities with them. In addition, she has participated in outreach to offer recovery opportunities in correctional facilities and rehabilitation centers. Stevie continues to have a servant heart for this community and looks for opportunities to be of service whenever possible. Stevie has lived in many different places across the U.S. as well as in Mexico and Spain and is able to speak conversational Spanish, increasing our capacity to serve the Hispanic, Latinx, Spanish-speaking, and immigrant populations.

Stevie holds a Bachelor of Science in Integrative HealthCare with a minor in Wellness Coaching from Metro State University in Denver. During her time in college, she studied a wide variety of complementary and alternative therapeutic practices which further ignited her passion for people, nature, and the healing arts. She continued her studies for a semester in Mexico, living in a tent in the Baja where she also received a 200-hour yoga teacher certification and a yoga Nidra (Sanskrit word for sleep) meditation facilitator certificate. She often shows her compassion and nurturing side through sharing home cooked meals and homemade herbal medicine. Stevie is incredibly grateful to be in Grand County working with and building up the recovery community.

Selena Handler, MSW
Lead Patient Navigator

Selena joined the Grand County Rural Health Network in May 2023 as a Care Coordinator, excited to be part of an organization that makes waves and moves mountains toward positive healthcare policy and improved accessibility in Grand County and beyond. In April 2024, Selena was promoted to the Lead Patient Navigator, continuing to serve some of our highest need clients while providing mentorship and indirect supervision to our patient navigation team. Selena has worked with survivors of intimate partner violence and their families for 7 years in both Wisconsin and New York City. Anti-racism and anti-oppression are practices at the core of her direct service work and have informed her advocacy against oppressive systems. Alongside her clients, Selena pushed for changes to benefits cliffs and housing policies that negatively impacted the community she worked within in New York City.

Before receiving a Master of Social Work from Columbia University, Selena worked as a Human Services Assistant at a Detoxification facility in Wisconsin while working at the only domestic violence shelter in the county. There Selena witnessed the link between trauma, violence, and substance use, and became passionate about applying a trauma-informed framework to her advocacy at all levels. After taking a course on Animal Human Health at the University of Denver, Selena decided to move to the mountains of Colorado to have better access to nature and animals that strengthen her whole-body health. She hopes to incorporate the connection between animal and human health into her work in Grand County.

Teresa Hood, HSAe

Teresa joined the Grand County Rural Health Network in May 2024 as the Resource Specialist and is honored to serve the community. As the resource specialist, Teresa will be fully focused on serving the needs of those navigating the complexities of the healthcare system and is deeply committed to education and advocacy for the advancement of health equity. Teresa firmly believes that by harnessing the collective power of education, advocacy, empathy, and collaboration, the Grand County Rural Health Network team can help lead the charge and pave the way towards a future where every resident can have the access to the health care and support needed to thrive.

Teresa earned her Bachelor of Business Administration from Texas Christian University where she also distinguished herself as a scholar athlete and member of the Women’s Golf team. Following graduation, Teresa began her sales and marketing career in the commercial flooring industry but quickly found her way to the health care industry, where she was better able to pursue her love of learning, educating and serving. Driven by faith, and a strong desire to care for her family and community, Teresa took a deliberate career hiatus to immerse herself in volunteerism, sports coaching, and community leadership. It was during these years that Teresa discovered the beauty of Grand County and formed an incredible love for the mountains. Teresa has been a part time resident of Grand County since 2001 but made the pivotal decision to make it her permanent home in 2020.

Teresa’s pure joy is spending time with her adult children and chosen family, but she is also an avid golfer, hiker, cross country skier and kayaker.

Kendal Flynn, MS
High Fidelity Wraparound Coordinator

Kendal joined the Grand County Rural Health Network in June 2024 as a High Fidelity Wraparound (HFW) Coordinator. In this role, she serves as the HFW Facilitator and the Family Support Partner. She is excited to help families in Grand County by using an evidence-based model of care for children and young people with behavioral health challenges. She brings years of personal and professional experience that give her insight into complex family systems and methods of care.

With a focus on advocacy, Kendal is passionate about helping people improve their well-being. She worked for 10 years as a special education substitute teacher and paraprofessional for a large education system in Virginia and Jefferson County, CO. She has followed her own son’s journey through the special education system and along the way, learned about the barriers to education for children with special needs.

Kendal has previous experience in Grand County as a Peer Support Specialist for Mind Springs Health and as a Job Coach through the Department of Vocational Rehabilitation. She participates in various recovery programs in the county and on the front range. She holds a master’s degree in Rehabilitation Counseling from the University of Wisconsin and worked with people who had disabilities at both Marionjoy Rehabilitation Hospital in IL and the National Rehabilitation Hospital in Washington, DC.

Kendal moved to Granby, CO from Virginia in 2018. She is a 5 th generation CO native and was born in Denver. She has lived in Wisconsin, Illinois, Washington D.C., Maryland and Virginia and is most relieved to be back in CO.

Kendal’s greatest joys are her two adult children and her pup named Josie. When she is not working, she seeks solitude in nature. You may cross paths with her on the slopes, on the trails, on her SUP, 4-wheeler or full suspension e-bike. Kendal also has an undergraduate degree in art and enjoys painting in her spare time. She uses her creativity to help think outside of the box on solutions to problems.

Katie Moon

Katie started with Grand County Rural Health Network in June 2024. She has an Associate of Arts degree in Psychology from Colorado Mountain College and is currently working towards completion of Bachelor of Arts degree in Human Services.

Prior to her work at Grand County Rural Health Network, Katie worked for XCEL Energy for 19 years. This included a Control System Specialist apprenticeship in IBEW Local 111, through Supervisory roles and into engineering.

Katie has volunteered with The Chance Phelps Foundation for veterans as well as Freedom Hooves equine therapy when they were in operation over the years.

She hopes her new role as a Recovery Coordinator will give her the opportunity to support others and make a difference in the lives of individuals, families, and her community.

Estefania Baray

Estefania joined the Grand County Rural Health Network in May 2024 as a Promotora de Salud. Estefania is proficient in both English and Spanish, with a rich background spanning over a decade in community work and customer service roles serving rural communities. She built a solid foundation as a Health Navigator at the Family Intercultural Resource Center and later as a Medicaid Enrollment Specialist at the Summit Community Care Clinic. Through this work Estefania increased her skills in navigating complex healthcare systems and advocating for underserved populations. Even though she was based in Summit County, all the help and resources throughout the work also served surrounding counties including Grand, Lake, Park, and Eagle clients.

Having recently relocated to Grand County, she is very excited to join the Grand Rural Health Network, driven by a passion to address the different needs of her new community. Through her experiences in the health sector, Estefania has witnessed firsthand the difficult barriers that individuals and families encounter when accessing healthcare services. This fueled her commitment to promote health equity and empower community members to lead healthier lives. Estefania is thrilled to continue to serve her new community through expertise, love and dedication, as she advocates for, supports and empowers the community to become, “Healthier together.”

In her free time, Estefania loves spending time with her husband and 5 boys. She is a full-time sports travel mom. She loves to travel and explore new destinations enjoying different cultures and scenery each place offers. She loves to visit Mexico at least twice a year to spend quality time with her family living in Chihuahua. One of her favorite hobbies is crocheting gloves, mittens, scarves, blankets and more for her family during winter season.

Cindy Molina, BSN

Cindy joined the Grand County Rural Health Network team as a Promotora de Salud (Health Promoter) in May 2024. Raised in Grand County, her Mexican American heritage is deeply intertwined with her identity. Fluent in both Spanish and English, Cindy earned her Bachelor of Science in Nursing from the Autonomous University of Chihuahua in Mexico. Beginning her professional journey in August 2020, she served as a nurse in a rural hospital located in the mountains of Mexico during the tough times of the COVID-19 pandemic.

As a rural nurse, Cindy dedicated herself to helping underserved communities, including the indigenous population of Chihuahua, Mexico. She offered vital health services to residents in distant areas to help improve both their work and living conditions. Cindy finds great satisfaction in providing support to those in greatest need, which makes her transition into this new role a happy one. Before working with Grand County Rural Health Network, upon returning to Colorado on 11/2022 Cindy worked for Fraser Medical Clinic, which was also a great experience. Cindy’s experience spans multiple specialties including obstetrics, critical care, pediatrics, emergency medicine, family nursing, operating room, and public health. Her dedication to her profession stems from her deep desire to help others.

As a Promotora, Cindy wants to empower the community to make informed decisions about their health, promoting an environment where everyone can live healthy and productive lives. She looks forward to helping people to navigate the health system, access available resources, and providing education and prevention to support healthy lifestyle practices, access to care, disease prevention, and self-care. Through this role, she will provide social support for individuals and families, helping them face health and well-being challenges.

Outside of work, Cindy enjoys hiking in the summer and being outdoors and immersing herself in the beauty of nature. Family gatherings hold special significance in her life, reflecting her cultural roots, she is the second of four siblings. Currently in her “dog mother” phase, Cindy cherishes her role as the loving parent to her two adorable Shih Tzus, Kai and Bruno.

Andy Lugo, MSW

Andy joined the Network in May 2024 as a Care Coordinator. He brings almost 20 years of comprehensive experience in social work and holds a Master of Social Work and a Bachelor of Arts in Science Education. He is also fully bilingual in English and Spanish. He has served in various capacities, including as a social worker at the Corporal Michael J. Crescenz VA Medical Center in Philadelphia where he provided direct clinical social work services focused predominantly on substance use disorder counseling, case management, and outreach for homeless veterans. His professional journey also includes roles as counselor at Delaware Valley University and Barry University where he provided mental health counseling, crisis intervention, and psychoeducational
workshops to students. Additionally, he has experience in education, having taught science at the middle school level in Miami, Florida.

Andy moved to Grand County in 2020 looking for a quieter and slower pace of life that’s closer to nature. What he loves the most about Grand County is that that he can look up at the night sky and clearly see the stars. In addition to looking at the night sky, he enjoys riding his mountain bike and playing video games.
